GOALS OF HSS

  • Assess wants and needs to create a tailored housing centered person focused plan

  • Support an individual’s transition into housing stability

  • Identify resources and effective technique for maintaining stability

  • Avoid future periods of homelessness or institutionalization

Meeting with Clients

TO QUALIFY FOR HSS
YOU MUST MEET THE FOLLOWING CRITERIA

  • 18 years of age and receiving Medical Assistance

  • Have a disability or a disabling condition

  • Be experiencing housing instability

  • Have a need for services due to limitations caused by individual disability

DOCUMENTS NEEDED FOR DHS ELIGIBILITY VERIFICATION REVIEW:

  • Professional statement of need OR

  • Medical opinion form OR

  • Proof of recipient of SSI or SSDI

  • MN Choices Assessment OR

  • Long term Care Coordination Plan OR

  • Coordinated Entry Assessment

HOUSING INSTABILITY

Members must also be facing housing instability to qualify for housing stabilization services. The member must meet one of the following criteria:

  • Homeless

  • At risk of homelessness (including could become homeless without continued housing services)

  • Institutionalized (currently or within the last 6 months)

  • Eligible for a waiver (a person with an institutional level of care is also deemed at risk of institutionalization)
