BASIC 245D

Our 245D program is a state licensed Home and Community Based Program, providing Basic Services to care recipients with an aim of supporting the their daily needs, personal preferences, and activities needed to accomplish their personal goals.

Individuals are active stakeholders in the development of care plans. This includes decision making and personal advocacy to support their choices for providers and methods of care delivery. The person receiving services and the care provider work together to identify what is important to and for the individual which includes recruitment and selection of qualified workers; such as family members, friends, co-workers, and neighbors.

The 245D program offers services that include:

• Individualized Home Supports (without training) for BI, CADI, CAC and DD waiver plans

• In-Home Respite for BI, CADI, CAC, EW and DD waiver plans and AC program

• Out-of-Home Respite for BI, CADI, CAC, EW and DD waiver plans and AC program

• Homemaker Services for CADI, DD and EW waiver plans and AC program

• Individual Community Living Support for EW Waiver Plan and AC program
